Report insurance fraud

Insurance fraud raises premiums for everyone who pays them honestly. Tell our Special Investigations Unit about a policy, application or claim you believe is not legitimate — you may do so anonymously.

Why it matters

What insurance fraud is, and what it costs

Insurance fraud is any act intended to obtain an undeserved benefit from an insurance transaction — a fraudulent application, a staged loss, or an inflated or fabricated claim. It is not a victimless act: every dollar paid on a fraudulent claim is a dollar added to the premium every other policyholder pays.

What it looks like

Examples by line of business

Personal auto

Staged accidents, exaggerated or fabricated injuries, and inflated repair invoices for damage that pre-existed the reported loss.

CPP & commercial property

Claim padding — adding undamaged or unrelated items to a legitimate property or theft claim — and applications that misstate a business’s operations to obtain a lower rate.

Workers’ compensation

Premium fraud through misreported payroll or misclassified class codes, and claims for an injury that did not occur at work or did not occur at all.

Producer & application fraud

“Ghost brokering” — an unlicensed party collecting premium and never remitting it or binding real coverage — and an application that omits a material fact to obtain a policy or a lower rate.

Reporting

How to report, and what happens next

Report by phone, in writing, or using the form below. You may report anonymously — we do not require your name to act on a tip, and we accept anonymous reports.

  1. You report what you know

Call 1-800-874-3368 and ask for the Special Investigations Unit, write to us, or use the form below. Give as much detail as you have — names, policy or claim numbers, dates — but a partial tip is still useful.

  2. The Special Investigations Unit reviews it

A dedicated investigator reviews every report, including anonymous ones, and decides whether it warrants a full investigation.

  3. We investigate

An investigation may include reviewing records, interviewing parties, and coordinating with law enforcement or the state department of insurance’s fraud bureau.

  4. We act on the result

Depending on what the investigation finds, we may deny or rescind a claim or policy, and we refer confirmed fraud to state and federal authorities for prosecution.

Specific language and required placement of this statement vary by state; the version that applies to your policy appears on your application or claim form.
